The source of this HP Down debuff is Fia, the Deathbed Companion. She is one of the many NPCs that you can meet inside the Roundtable Hold and offers you one of the most valuable things that you can be offered at the end of the world: a hug. Hugging Fia will give you an item called the Baldachin’s Blessing, an item that can be used to increase the player’s poise for a short period of time.

This is also the reason that you have the HP debuff. As long as you have this item in your inventory, you will have the HP debuff. To remove the debuff, simply use the item and wait for the effect to wear off. If you are willing to sacrifice some health in order to get some extra poise, you can get a new Blessing by interacting with Fia again.

The icon sits beneath the Stamina bar, indicating an active Status Effect on the player. The Red Square Debuff is a direct result of the player embracing Fia, an NPC that Tarnished will meet in a small bed-chamber within the Roundtable Hold. When Elden Ring fans first encounter Fia, she will hold a brief conversation that ends with the dialogue options of the protagonist choosing to hug or not embrace her. Accepting the seemingly innocent offer will trigger a short animation of the player character kneeling down to allow Fia to hold them in her arms. What players may not realize is that the moment she presses their head to her chest, their HP bar in the upper-left corner will slightly deplete. Therefore, hugging Fia grants players the ability to invoke several seconds of higher Poise while sacrificing a minute yet significant portion of their health. Even 5% HP can make a huge difference in surviving a fatal blow; thus, players must ensure they understand what they are giving up in exchange for the Baldachin's Blessing. On the other hand, players can accept Fia's embrace for free emotional support and immediately use the restorative blessing afterward. After all, Elden Ring shows no mercy to Tarnished players.
